Part 1: Disclamers

Children's Registration Disclamers

Please Note:

  • This registration does not guarantee admission into the children's programs. Admission will be permitted at check in based on seating availability.
  • Admission into the children's programs will available from 9:30am on Sabbath. The programs will start at 10:00am.
  • Registration & admission into the children's programs will close (when room capacity is full) or at 12:00pm (whichever comes first).
  • All children's programs will conclude at 1:00pm. Kindly and promtly pick up your child at that time. A SMS message and various visual aids in the other meeting rooms will also notify you when the children's program has ended.
This document serves as the sole identification for Children's Ministries staff and volunteers regarding the adult authorized to pick up your child. As the parent / legal guardian / accompanying adult, you bear the sole responsibility for ensuring that the individual named on this document is indeed the designated person to collect the child specified herein at the conclusion of the program. The Ontario Conference of Seventh-day Adventist assumes no responsibility for the loss or misuse of this document. Any use of this document is entirely at your own risk. The Ontario Conference of Seventh-day Adventist shall not be held liable for any damages, whether direct, consequential, incidental, special, or arising from any claim whatsoever, resulting from the utilization of this document.
We make every effort to accommodate children with special needs. Due to staff and safety restrictions, however, there are limits to the services we can provide.
In case of emergency, I hereby give permission to medical personnel to secure proper treatment for this child and to order hospitalization, and medical/surgical intervention as necessary. I also release the Ontario Conference of Seventh-day Adventist from any and all liability in case of accident or illness.
This registration grants us permission to use your child's picture in promotional material.
